Shirts will be navy blue with this design on the front.

Options for shirt styles are as follows:

• Regular T-shirt (short-sleeved, pre-shrunk)
• Fitted Girls' T-shirt (short-sleeved, NOT pre-shrunk, will shrink a lot)

Ordering instructions:

2. Complete the form with your child's name and teacher name, sizes, amounts and total amount due
3. Enclose completed form and check in an envelope marked "PTA" (if you would like to pay with credit card, see online option below.)
4. Send to school with your child or return to the office
5. Orders are due by September 2nd.

All adult-sized orders must be in by Monday Dec. 9th2013, or 10 am Tuesday at the
latest, so that they will be back before winter break
